The Liberty CLC is about you!

It is a place where we are connecting to our neighborhood community. We know each other because we are involved and engaged in educating ourselves, each other and all children. We achieve this by celebrating diversity, health, and wellness with fun learning activities for people of all ages.

Liberty Community Health Clinic: Intermountain Health Care (IHC) is open to Liberty students, families, and community. Services provided to insured and uninsured, based on a sliding fee scale.
Hours of Operation: Monday through Friday 7:00 am to 3:30 pm

Liberty Community Dental Clinic: The University of Utah School of Dentistry is open to Liberty students, families, and community. Services provided to insured and uninsured, based on sliding fee scale. All initial pediatric visits are $35.00.
Hours of Operation: Monday through Friday 8:00 am to 5:00 pm

Valley Behavioral Health Services: All Liberty students receiving services must be referred through the school to receive one-on-one mental health support from Ms. Julie. If you have a child in need of additional support please contact his or her teacher directly to be connected to school counselor and resources available.
Hours of Operation: Monday through Friday 8:00 to 3:00 pm  
Friends for Sight: Free vision screenings for all ages, Wednesdays from 9:00 to 12:00 pm.
Vision Clinics: Calendared throughout the school year with a practicing ophthalmologist or pediatric ophthalmologist.

Salt Lake County - AUCH Community Health Worker Team: Who we are and What we do? (Resource Navigator) We help you access community resources including food, legal aid, utility assistance and more. (Connector) We connect you with affordable care and help you through the healthcare system. (Listeners) We are part of your community and understand your concerns. (Problem Solvers) We listen to your needs and help you find real solutions. (Wellness Advocates) We help you maintain or improve your health by setting goals or keeping current ones.
Hours of operation: CHC Case-workers are available Wednesdays and Thursdays from 8:30 am to 3:30 pm in office #1059.

Division of Workforce Service (DWS): On-site services available to the community. If you need to apply for Medicaid, CHIP or Emergency medical assistance or if you have an open case and need to check on the status or submit additional paperwork come in. No appointment necessary. Walk-ins Welcome!
Hours of operation at Liberty: Monday, Wednesday, and Thursday from 8:00 to 3:00 pm
Hours of operation at Horizonte School: Tuesday and Friday from 8:00 to 3:00 pm.
Community Food Pantry: Liberty has an established partnership with Utah Food Bank and private donors to host its own food pantry. Although supplies are limited to nonperishable goods, we can meet emergency food needs for students, families and community members. If you or someone you know are in need please feel free to come in, no appointment necessary.
Hours of Operation: Tuesday and Thursday from 2:30 to 5:30 pm
Women's Support Group:  IHC provides a free session directed by a psychologist  once a month for women in the community. We will talk, listen and share the daily problems we have to confront as women. This group is offered the third Wednesday of each month. Join us Wednesday, Nov. 20, 2019 from 11:30 to 1:00 pm in CLC Conference Room #1065.

Monthly Mobile Food Distribution & Give-a ways:  Utah Food Bank provides Liberty families with a mobile food distribution day each month, the first Wednesday of a full week in each month. Food items are distributed peer family, each family is encourage to participate. The give- a ways are provided by community donations only, all donations received with be distributed once a month on a first come first to take basis. All remaining items at the end of distribution will be donated to the Big Brother Big Sister Organization.

ADULT GED PREP CLASSES: Liberty is offering two classes per day, Monday through Thursday, Beginning Course 9:00 to 12:00 and Intermediate Course 1:00 to 4:00 pm. Participants will register at Horizonte but attend Liberty CLC Rm #1024.
Adult English Class: TBA – We are currently looking for an instructor to teach evening classes. Classes will be offered free to the community.
Diabetes Class: Class will be held on Tuesday’s Nov. 5 and Nov. 12 from 6:00 to 8:00 pm in room #1023. Classes are offered by IHC to the community free of charge, everyone is welcome to attend. 
Yoga Class:  Tuesdays and Thursdays from  6:00 to 7:00 pm Fall Session (1) begins November 12 and ends December 19, 2019
Zumba & Healthy Blends Class: TBD – We are currently looking for an instructor to teach. Must have liability insurance! If you or someone you know is interested in teaching please contact us immediately! Liberty’s Zumba Nation is ready and waiting for you to begin.
Afterschool – Kids in the Kitchen:  Fall Session is scheduled to begin on Tuesday, November 5 from 3:10 to 4:10 pm for Groups 3 & 4 and 4:15 to 5:15 pm Groups 5 & 6. Classes provided by Utah 4-H program. 

Bad Dog Arts: Multi-Cultural Art Program will begin November 1, 2019. Students will be given a chance to participate in a visual art program after school. Class will be offered on Fridays for 8 weeks to students grade 3,4,5 and 6.  Beginning the new year students will learn more about variety of dances from countries and cultures. Goal is to have a multi-cultural celebration at the end of the school year for families to come and enjoy.



Parents & Pastries will now be called Parent Meeting. The first Parent Meeting will be held on Friday, Nov. 15 from 8:15 to 9:15  in the Liberty Elementary Cafeteria. If you have questions or concerns please contact Claudio Torres by email


Community Awareness Events:

1) Free Community Zumba “Family & Me” Wednesday, November 13 from 6:00 to 7:00 pm in the gym.

2)Our first event will be Wednesday, November 20 from 6:00 to 7:00 pm in the CLC. Vaping Awareness: We know so many families and people we know are effected by this and want to provide SLCSD parents and guardians more information. Please come join in the conversation!
